Our Referral Process
At Guided Pathways, we welcome referrals for children and young people who require a safe, supportive residential placement. Our referral process is clear, collaborative, and centred on ensuring the right match for each young person and our home environment.
We work closely with placing authorities and professionals to carefully consider each referral, with a strong focus on quality, safeguarding, and positive outcomes.
Who We Support
We accept referrals for children and young people with:
Emotional and Behavioural Difficulties (EBD)
Social, Emotional and Mental Health (SEMH) needs
Experiences of trauma, attachment disruption, or instability
Difficulties engaging with education or daily routines
Needs that benefit from a small, structured, relationship-based setting
All referrals are considered on an individual basis
We Follow These Process Steps:
Initial Enquiry
Referrals can be made by local authorities, social workers, or commissioning teams. Initial discussions help us understand the young person’s needs, background, and placement objectives.Information Review
We review all available documentation, including assessments, risk information, and care plans, to ensure we can meet the young person’s needs safely and effectively.Matching & Planning
Each potential placement is carefully matched, considering the young person’s needs, the dynamics of the home, and staffing arrangements. This ensures placements are well-planned and sustainable.Decision & Next Steps
A clear and timely decision is provided. Where appropriate, we work collaboratively to plan transitions and support a smooth move into the home.
Information We May Request
To support effective decision-making, we may ask for:
Recent assessments and care plans
Risk assessments and behaviour support strategies
Education and health information
Placement objectives and timescales
Any relevant professional reports
